OHS forms are always good for 'Identified Hazards'.. - If its a hazard to your safety, document it correctly. You will be surprised how powerful the OHS push can be these days.. (Its all about 'butt covering'..)

Heh.. Yeah, isn't it unfortunate, that the only reason we are kept safe, is so people can 'cover' their own behinds, to avoid the risk of prosecution if something were to happen.
